Do Not Resuscitate (DNR) order
A DNR Confirmation Form provides direction to both paramedics and firefighters in situations where a patient has an expressed their wish not to be resuscitated in the event that they experience cardiac arrest. It is a legal document that must be completed in full and handed directly to arriving emergency responders.
Please be aware that if emergency medical service personnel are attending to a non-responsive person who is assessed to have no pulse present or signs of breathing, CPR will be performed unless EMS are handed a DNR order form.
Paramedics and firefighters may provide interventions or therapies considered necessary to provide comfort or alleviate pain. Please refer to the DNR form for more information.
Where can you obtain a DNR form?
Check with your family physician for more information. This form cannot be printed or completed on-line due to the unique serial number that is pre-printed on each form. For the form to be complete it requires the signature of a physician (MD), registered nurse (RN), or registered practical nurse (RPN).
What is a Medi-tube?
SAFEsystem’s Medi-tube Cylinder is a 12″ x 1″ watertight container providing a list of vital information to Emergency Responders about each resident at that location in the event they cannot speak for themselves. The cylinder is stored in a convenient area in the home and may be updated by the residents at any time. The combination of both the SAFEsystem residential lock box and Medi-tube cylinder are involved in reducing the time for EMS personnel to enter a home and access personal information to serve you better.

The SAFEstick is a memory stick that is attached to the lock mechanism of your lock box. It contains specific contact information that the emergency responders can utilize to reach someone after hours, in the case of an industrial/commercial customer, or when no one is at home in a residential situation.
